Safety Incident Disclosure

We disclose major safety incidents and dangerous queries and show how Max responds only when it's safe for us to do so, and this disclosure is in the public interest, such as when a vulnerability has been identified and resolved. Many incidents are handled internally, and not published. We publicly disclose incidents up to 3 months after they occur.

December 3 2025 — Significant Safety Fault

Emergency Guidance fault

We have observed a significant deviation from Max's protocols when an Emergency Guidance trigger was activated. Instead of issuing a questionnaire, Emergency Guidance immediately delivered steps without knowing necessary context.

Max Generation 3

This fault has been observed on Max Generation 3 only. No other systems are impacted. Such a fault is rare, and this is the first time in over 170 Emergency Guidance tests and incidents we have observed such behaviour.

Taking steps to prevent this

We've now conducted an internal review of the incident and have released an emergency update for December 4th. We have updated Max's protocols and Foundational Safety Framework. We'll continue to monitor for similar incidents in the future. We expect the likely-hood of a similar event to be low, but not unlikely.
